We provide medical certificates for work, school, study, and other official requirements.

Get a medical certificate

If you need a medical certificate for time off work or study, our team can help. Certificates are provided when clinically appropriate, and you may need an appointment so that we can assess your health and support your recovery.

 

We can provide certificates for:

  • Work or employer requirements
  • Sick leave
  • School or university absence
  • ACC-related injuries
  • Insurances or other official requirements
  • Fitness to return to work or activities
  • Travel or event cancellations (where appropriate)

Book an appointment

Certificates will often require a consultation with a doctor to confirm your illness, injury, or fitness for activities.

  1. Request a certificate through your Local Health App.
  2. Book an appointment through the Local Health App or phone us on 06 357 8061

Collecting your certificate

Your certificate can usually be:

  • Sent electronically
  • Printed for collection at reception
  • Sent directly to your employer (with your consent)
